Understanding the I-693 Form: A Comprehensive Guide for Immigrants

What is i-693 or what is i-693 application form?

The i-693 form, or Report of Medical Examination and Vaccination Record, is required for applicants seeking an adjustment of status to become a lawful permanent resident in the United States. The form is used to report the results of medical examination that establishes that you are not inadmissible to the United States on public health grounds. It also serves as record of required vaccinations.

You should only go ahead to undergo the medical examination when you are instructed by the U.S Citizenship and Immigration Services ( USCIS) as the results are only valid for one year.

There are designated Civil Surgeons authorized by USCIS to carry out these tests. The USCIS website has a list of the authorized civil surgeons.

Based on your vaccination history, the civil surgeon is required to administer necessary vaccinations to meet the US vaccination requirements.

The i-693 form and supporting documents are to be submitted to USCIS in a sealed envelope which you must not open.

The Structure Of The I-693 Application Form

The i-693 form is a 14 page form and has14 parts.

Part 1 of the i-693 form contains information about you, this part should be filled by YOU, the person requesting the medical examination, and not the Civil Surgeon.

Part 2 of the i-693 form contains the Applicant’s Statement, Contact information, Certification and Signature.

Part 3 of the i-693 form contains the Interpreter’s Statement, Contact information, Certification and Signature.

Part 4 of the i-693 form contains the contact information, Declaration and Signature Of The Person Preparing The Application, if it is not the applicant.

Part 5 of the i-693 form contains the Applicant’s Identification Information (This part will be completed by the Civil Surgeon).

Part 6 of the i-693 form contains Summary Of The Medical Examination (This part will be completed by the Civil Surgeon).

Part 7 of the i-693 form contains the Civil Surgeon’s Contact Information, Certification And Signature.

Part 8 of the i-693 form contains the Civil Surgeon’s Worksheet.

Part 9 of the i-693 form contains Referral Evaluation.(To be conducted by the health department or other doctor performing the referral evaluation).

Part 10 of the i-693 form contains Vaccination Record.

The i-693 Medical Examination Service Fee

The I-693 Medial Examination service fee is $400. Included in this cost is payment for STD and Tuberculosis testing via Quantiferon blood test as required by USCIS. If you are known to have positive tuberculin results, you are still required by USCIS to take the Quantiferon test. However, the $400 payment does not cover the cost of the additional Chest X-Ray that will be required following a Positive Quantiferon Test result.   The $400 also does not cover the cost of any vaccinations that you may need. The $400 only covers the Physician’s examination, STD screening, and the Quantiferon Test.
